The 25-year-old Cishek entered the final inning with a 6-4 lead, but surrendered an RBI double to Michael Martinez before buckling down to get the last two outs and earn Florida the triumph.
"We keep it entertaining, no question about that. We make it an interesting game every night," Marlins manager Jack McKeon told reporters. "Thank goodness we were able to hold on."
The Marlins (59-72), last in the National League East, had lost seven of eight contests heading into the night and remain 25 games behind the Phillies (83-46) in the division.
Catcher John Buck sparked Florida by blasting a grand slam in the sixth inning to give the visitors a 6-1 lead.
Florida starter Clay Hensley got the victory after he was charged with three runs in 5 2/3 innings.
He exited i n the sixth just before Ryan Howard drove a three-run shot out of the ballpark to quickly pull the home team within 6-4 and put further pressure on the Marlins' relievers.
"It was definitely an incredible rush like I've never felt," Cishek said, of earning his third save of the season.
"I've never heard anything so loud in my life. So being able to block that out and go after hitters meant a lot."
Philadelphia's Roy Oswalt had a rough outing for Philadelphia, and took the loss after allowing six runs and 12 hits in 5 2/3 innings.
Oswalt (6-8), who has missed 37 games this season with injury, was booed by the home crowd when he was relieved.
Greg Dobbs went 4-for-5 at the plate for Florida and had an RBI double in the fourth where the Marlins went ahead 2-1.
Wilson Valdez homered for the Phillies in the third.
